According to the Consular Section of
the Ukrainian Embassy

post-placement reports "should be submitted once a year for the first
three years after adoption, and then once every three years until the
child reaches the age of 18. The Consular Office of the Embassy of
Ukraine will be grateful for your detailed reports about adopted
child's (children's) living conditions and development and pictures of
your family." Click the link below to download and complete the
post-placement report form provided by the Embassy of Ukraine.
Please note: The US Department of State
has also confirmed that a letter from the parents which includes 10-15
photos will also suffice.
The reports should be sent by mail to the appropriate consular office
or consulate for the region where the child and family reside (e.g. a
Virginia family would send its report to the consular office at the
Embassy, a California family to the Consulate General in San
Francisco, an Illinois family to the Chicago Consulate General, etc).
